Service Coordinator - Intellectual Developmental Disability (IDD)
Role Responsibilities
The IDD Service Coordinator manages services for participants in one of the following programs : General Revenue, Community First Choice, Texas Home Living, or Home and Community Based Services. The Service Coordinator works under the supervision of the designated program Team Leader, IDD Authority Services Division. The IDD Service Coordinator is responsible for the following :
- Assessment, service planning, monitoring, advocacy and crisis prevention and management for assigned caseload of individuals with IDD.
- Provision of Medicaid billable targeted case management activities.
- Compliance with applicable local, state and federal rules and regulations.
- Adherence to and promotion of the Texana Compliance Policy.
- Travel within 6 county regions.
- Work hours : M-F, 8-5 with flexibility based on client need.
Salary
$20.70 to $25.25 / hour based on years of experience performing Medicaid Targeted Case ManagementRequired Education and / or Certifications, Experience, Skills
Bachelor’s or Advanced degree from an accredited university or collegeTexas Driver License (in good standing)Service Coordinator I : No experience required.Service Coordinator II : Two years as Service Coordinator, or equivalent capacity in IDD field.Service Coordinator III : Four years as Service Coordinator, or equivalent capacity in IDD field.Service Coordinator IV : Six years as Service Coordinator, equivalent capacity in IDD field.Preferred Education and / or Certifications, Experience, Skills
Strong organizational and computer skills (Microsoft Office and Electronic Health Record system).Knowledge of Service Coordination State Rules.Experience working with individuals with IDD.Job related Physical Requirements
Must be able to operate a vehicleMust be able to perform SAMA and CPR.
