Our Client, a Healthcare company, is looking for a RN - Case Manager for their Fort Wayne, IN location.
Responsibilities :
- Collaborative work with the Care Team.
- Completion of comprehensive patient assessment including patient status to drive discharge.
- Driving stay progression
- Completion of discharge planning in a manner that expedites patients transition to the next level of care.
- Comprehensive and accurate Epic documentation
