Job Title : Registered Nurse (RN) – Home Health

Specialty : Home Health

Address : Grand Rapids, MI

Shift : 5 x 8 Day Shifts

Duration : 3 Months Contract (with possible extension)

Pay Rate : $55.00 per hour

Guaranteed Hours : 40 hours per week

Position Overview

We are seeking an experienced Home Health RN to join our client’s team in Grand Rapids, Michigan. This contract position offers competitive pay, guaranteed hours, and the opportunity to work with a leading home health organization.

Responsibilities

Provide skilled nursing care in a home health setting
Travel to patient homes within the coverage area : Howard City, Six Lakes, Tufant, Cedar Springs, Rockford, Belding, and Saranac
Perform patient assessments, develop and update care plans, and coordinate with healthcare teams
Educate patients and families on disease management and health maintenance
Document visits and care in compliance with regulatory standards

Requirements

Active Registered Nurse (RN) license – Michigan State Board

Minimum 1 year of nursing experience (Home Health experience preferred)

Strong clinical, communication, and organizational skills

Ability to work independently and manage patient visits across multiple counties

Equipment & Software Needed

Tablet, phone, nurse bag (provided or required per facility policy)

Access / knowledge of : Forcura, Swift, HCHB
