Administrator / LPN

Conduct in-home assessments and reassessments for clients in accordance with state and agency guidelines.

Develop individualized care plans in collaboration with the care team.

Document findings accurately and promptly in the agency’s electronic system (AxisCare).

Communicate with clients, families, and caregivers to ensure client satisfaction and safety.

Monitor client condition changes and update care plans as needed.

Maintain compliance with all HIPAA and LDH requirements

Qualifications

Bachelor’s degree (preferred).

Current Louisiana LPN license in good standing (required).

3–5 years of clinical or home health experience preferred.

Strong assessment, documentation, communication, and computer skills (experience with electronic charting systems such as AxisCare preferred).
