Position Summary :
The Advanced Care Team (ACT) Clinician provides clinical insight and education to patients served by CPAP Central (CPC ) . This role includes patient instruction on the safe and appropriate use of home respiratory equipment and other medical devices. The ACT Clinician plays a critical role in supporting the continuum of care through patient education, follow-up, care coordination, and compliance with applicable regulations.
Schedule : Monday-Friday 8am-5pm; no holidays or weekends!
Essential Functions :
Patient & Caregiver Education :
- Educates patients, families, and caregivers about diagnoses, equipment use and care, symptom recognition, and personal care through phone calls, Telehealth visits, and in-person interactions as needed.
Clinical Follow-Up :
Conducts daily follow-up tasks including general patient check-ins, non-invasive ventilation (NIV) follow-ups, and communication with patients or caregivers.NIV Services :
Performs NIV setups and in-home visits as necessary. Participates in on-call rotation as assigned.Care Coordination :
Supports the continuum of care by creating and managing care plans, communicating with physicians and clinicians, and proactively resolving patient care issues.Product & Service Knowledge :
Maintains thorough knowledge of equipment and services. Assists with equipment quality control, maintenance, and Clinical Outcomes Team operations.Insurance Education :
Understands and communicates patient insurance coverage, benefits, and out-of-pocket costs to patients and / or caregivers.Regulatory Compliance :
Adheres to HIPAA, Joint Commission standards, State Respiratory Care and Pharmacy Board regulations, and other applicable guidelines.Licensure & CEU Maintenance :
Ensures active professional licensure and meets continuing education (CEU) requirements.Additional Duties :
Performs other tasks and responsibilities as assigned by the manager.Qualifications :
Active LPN or RN license in stateAt least one year of experience as an LPN or RNHome health experience preferred
