• Referral Program Requirements : \n
• Minimum 1 year of Home Care Experience\n
• Strong knowledge of current nursing practices.\n
• Understanding of team and primary care nursing concepts.\n
• Familiarity with the OASIS process.\n
• Awareness of community resources.\n
• Skilled in making meaningful observations and evaluations.\n
• Ability to maintain therapeutic relationships with patients and healthcare teams.\n
• Competence in providing skilled nursing care based on scientific principles and nursing processes.\n
• Proficient in reading, writing, and speaking English clearly.\n
• Physical ability to perform nursing duties, including lifting and moving patients.\n
• Nursing degree from an accredited institution and graduate of a CT approved RN program.\n
• Valid driver's license and car insurance.\n
• Active RN License in the State of CT.\n
