Job ID : 135202\n\nDirect Hire Registered Nurse Role $44.66-$58.08 per hour in Sacramento, California\n\nClinical Magnet (part of ICON Medical) is looking for a compassionate Eligibility RN to join their In-Home Services team in Sacramento, California.\n\nThis is a DIRECT HIRE, full-time opportunity, onsite.\nResponsibilities of the Eligibility RN : \n

• Schedule and conduct home visits to assess prospective participants and determine level of care.\n
• Complete documentation in collaboration with primary care physicians and the enrollment team.\n
• Participate in outreach programs to support community engagement.\n
• Serve as a member of the Interdisciplinary Team (IDT), coordinating additional assessments as needed.\n
• Provide consultation to case managers and staff on medical and nursing issues.\n
• Maintain knowledge of Medicare, Medicaid, and PACE program eligibility guidelines.\n
• Assist with in-home assessments, reassessments, supervisory visits, and routine RN care as scheduled.\n\nQualifications of the Eligibility RN : \n
• Associate degree in Nursing from an accredited school.\n
• Current RN license in the state of practice.\n
• Current First Aid and BLS certifications (American Heart Association or Red Cross).\n
• 3+ years of healthcare experience with emphasis in geriatrics.\n
• 1+ year working with frail or elderly populations.\n\nCompensation and Benefits for Eligibility RN : \n
• $44.66 – $58.08 per hour (based on experience).\n
• Full-time, onsite schedule.\n
