Job Requirements

Minimum of 1 year of experience in Case Management or Homecare

Valid RN License

Strong communication and organizational skills

Responsibilities

Coordinate and manage patient care plans in a homecare setting

Conduct patient assessments and develop individualized care plans

Collaborate with healthcare providers and family members to ensure optimal patient outcomes

Monitor patient progress and adjust care plans as necessary
