State Licensed Post-Acute Care Manager

A company is looking for a Manager of Post-Acute Care Operations - Population Health.

Key Responsibilities

Develops and oversees the professional practice model for post-acute care management

Coordinates care across various post-acute sites and acts as a point of contact for stakeholders

Participates in quality improvement initiatives and integrates programming into care delivery

Required Qualifications, Training, and Education

Current RN License in the state of practice or covered by compact (required)

Bachelor of Science in Nursing (BSN) (required)

3 years of experience in transitions of care or care management (preferred)

Care Management certification (preferred)

Master's degree in Nursing or Healthcare Administration (preferred)
