Arizona Licensed Pediatric Nurse Manager

A company is looking for a Newborn / Pediatric Nurse Care Manager who is passionate about caring for members holistically through their healthcare journey.

Key Responsibilities :

Deliver coordinated, patient-centered virtual Care Management by telephone and video to improve members' health outcomes

Create impactful care plans with members and the care team to help achieve desired health goals

Assist members in navigating complex medical conditions, treatment pathways, and the healthcare system

Required Qualifications :

Bachelor of Science in Nursing (BSN)

Current CCM Certification or eligibility within 6 months of hire

5+ years of experience in nursing and 2+ years in care management

Active Compact RN license in good standing and must reside in a compact NLC state

Experience working remotely and strong competence with computer / medical record systems
