• Filled prescreen form attached upon submission.
• Resume and license verification attached.
• References (3) required.
• County Criminal Check (7 years for all counties lived and worked).
• Federal Criminal Check (7 years or more).
• Multi-State Criminal Search (7 years or more).
• Nationwide Sex Offender Search (7 years or more).
• HHS / OIG Search.
• GSA Search.
• Social Security Number Trace.
• PPD or Quantiferon test or proof of positive PPD and chest X-ray.
• SAM Search.
• DOJ Check (reran every 4 years with BID form).
