Salary : $25 $30 per hour

Stallant Health & Wellness is inviting applications for a Referral Coordinator to join our newly opened Rural Health Clinic in Highland, CA. This role is essential in ensuring seamless coordination of specialty care, supporting both our patients and providers by managing referrals with accuracy, efficiency, and compassion.

We are seeking a Referral Coordinator who is detail-oriented, organized, and proactive, but who also shares our vision that the way care is delivered matters just as much as the care itself. The ideal candidate will bring strong communication skills, a collaborative spirit, and a genuine passion for helping patients navigate the healthcare system with confidence and ease, all while contributing to a warm and welcoming clinic environment.

Duties would include :

  • Coordinate and process referrals for patients to specialists, diagnostic services, and other healthcare providers.
  • Ensure timely and accurate submission of referral requests, prior authorizations, and follow-up documentation.
  • Communicate with patients regarding referral status, appointment scheduling, and next steps.
  • Collaborate with providers, nurses, and clinic staff to ensure smooth transitions of care.
  • Maintain accurate records in the electronic health record (EHR) system.
  • Act as a patient advocate by assisting with questions, clarifying instructions, and supporting access to needed services.
  • Monitor referral outcomes and ensure completion of specialist reports back to the clinic.

Qualifications :

  • High school diploma or equivalent required; associates or bachelors degree in healthcare administration or related field preferred.
  • Previous experience in a medical office, clinic, or healthcare setting strongly preferred.
  • Knowledge of insurance requirements, prior authorizations, and referral processes.
  • Proficiency in electronic health record (EHR) systems and Microsoft Office Suite.
  • Strong organizational skills with attention to detail and accuracy.
  • Excellent interpersonal and communication skills, with the ability to interact effectively with patients, providers, and external offices.
  • Ability to work independently while thriving in a collaborative, team-oriented environment.
  • Bilingual (English / Spanish) preferred but not required.
  • Salary

  • $25 $30 per hour, depending on experience.
