Responsibilities
- Assist with personal hygiene, including bathing, grooming, toileting, and dressing
- Provide meal preparation, light housekeeping, and medication reminders
- Support mobility and transfers, using gait belts or Hoyer lifts if trained
- Offer companionship and emotional support to the client and family
- Communicate any changes in condition to supervisor or nurse case manager
- Maintain a clean and safe home environment for the client
- Accurately document care tasks completed each shift
