POSITION SUMMARY :
Provides health care to the geriatric population in a specialized program and performs selective medical services within the Skilled Nursing Facility (SNF) setting through a collaborative agreement with a supervising MD / DO. Responsibilities include conducting annual wellness visits, along with other routine and specialized assessments, to ensure comprehensive care for older adults.
ESSENTIAL FUNCTIONS :
- Obtains complete medical history and physical examination on patients.
- Reviews medical records.
- Interprets and integrates data to determine appropriate diagnostic and therapeutic procedures.
- Establishes presumptive diagnosis.
- Orders appropriate laboratory and diagnostic procedures.
- Develops a therapeutic plan for patient.
- Sutures wounds.
- Prepares written prescription orders for drugs and controlled substances.
- Instructs patients and family regarding medications and treatment instructions.
- Provides recommendations regarding health and illness prevention.
- Recommend community resource to meet patient / family needs.
- Performs independent evaluations and treatment procedures in emergency situations.
- Provides monitoring and continuity of care between physician visits.

KNOWLEDGE, EDUCATION AND SKILL REQUIREMENTS :
3-5 years Primary Care Practice Experience or geriatric setting preferredGraduate of an accredited Acute Care Nurse Practitioner program.Licensed for Nurse Practitioner license in the state of Vermont.Strong inpatient skill set.Quality-focused team player.Excellent organizational, time management and problem-solving skills.Ability to function effectively as part of a multi-disciplinary team.Effective oral and written communication skills.Commitment to providing exceptional patient care.BENEFITS :
