Care Coordinator Supervisor
The position is responsible for upholding RHD values, adheres to the standards of DDAP, OMHSAS, Magellan, Medicare, and any other funding or licensing bodies. The care coordination supervisor oversees a team of care coordinators in order to ensure that services are being provided according to program specific guidelines and policies. The supervisor assures that the delivery of services is robust and recovery and resiliency oriented. The supervisor assesses the quality of services being delivered and develops change strategies as needed in order to empower employees to support participants in the pursuit of their goals.
Section 3 : Major Responsibilities
- Ensuring that all practices align with program standards.
- Hiring, training, and supervising all new staff on the Care Coordination team.
- Coordinating continuing education trainings for employees. Taking on personal development and building a learning environment for the team.
- Attending and / or facilitating trainings in support of current or future services.
- Reviewing individual cases with team to develop an approach to challenges and facilitate communication with others.
- Tracking, promoting, and maintaining employee engagement through individual, group, and field supervision. Mentoring staff in the areas of professionalism, crisis management, effective communication, and professional growth.
- Engaging, managing, screening, assigning, and tracking all Care Coordination referrals.
- Reviewing and approving progress notes and hospitalization logs in the electronic health record (EHR) by providing careful review of service delivery and accuracy.
- Developing and utilizing various reports to reflect and monitor relevant information and measure program outcomes, such as tracking services provided to participants, open cases, termination of cases, productivity, compliance, etc. Completing ongoing reporting requirements.
- Ensuring the coordination of treatment planning and discharge planning activities for individuals admitted to inpatient care. Monitoring the completion rate of aftercare appointments. Completing a clinical review for the admission to identify needs and prevent future admissions.
- Coordinating and leading treatment team meetings and Root Cause Analysis (RCA) meetings.
- Modeling engagement skills and fostering positive relationships with individuals in service.
- Attending County meetings to stay current with current trends, opportunities, and barriers while positively representing RHD / LMCMS.
- Collaborating with County representatives and other providers to promote RHD / LMCMS and maintain positive affiliations.
- Assisting the Agency with creating and maintaining a welcoming environment for both individuals in service and employees.
- Participating in marketing activities as needed.
- Educating stakeholders regarding the Care Coordination process and health-related issues.
- Proactively identifying individuals who have multiple or complex medical, social, and / or psychiatric needs or individuals who are at risk of developing complex needs during an acute episode of illness.
- Collaborating with RHD program staff and outside providers to coordinate care across the spectrum of health services, including behavioral health care, as well as social services, housing, educational systems, and employment opportunities as necessary to facilitate wellness and recovery of the whole person.
- Submitting Dynamics request for Montgomery County individuals to obtain an MHX number.
- Ensuring all ACM clients have an MHX number, are admitted to the EHR under ACM episode, and that Montgomery County funding is established

Strong knowledge of community resources, hospital admission and discharge processes, recovery principles and language, and peer support practices and values.FBI, Child Abuse, and PA Criminal History ClearanceValid driver’s licenseEducation and Experience :
A Bachelor’ s degree in sociology, social work, psychology, counseling, education, gerontology, anthropology, criminal justice, or related area. Master’s Degree Preferred.5 years mental health direct care experience, and supervisory experience preferred.Job Competencies
