Job Title : RN Case Manager – Flexible Schedule + Growth Opportunity

Company : Home Health Care, Inc.

Location : Southwest Minnesota Region

Schedule : Full-Time (M-F, 8 : 00 AM – 5 : 00 PM), 1 weekend / month, 1 on-call shift / week

Compensation : $85,000 – $130,000 annually + $8,000 in first-year bonuses

What You’ll Do :

Perform assessments, develop care plans, and conduct skilled nursing visits

Serve as liaison between care providers, clients, and family members

Educate clients and caregivers on chronic disease management and safety

Ensure full compliance with OASIS and state / federal regulations

Qualifications :

Current MN RN license

Minimum 3 years of RN experience

Familiarity with home care documentation and regulations

Valid driver’s license and reliable vehicle
