Responsibilities
Manage patient care plans and coordinate services for optimal outcomes.
Conduct assessments to identify patient needs and develop care strategies.
Achieve performance targets and contribute to team success.
Collaborate with healthcare teams to ensure comprehensive patient support.
Participate in professional development and training opportunities.
Monitor patient progress and adjust care plans as necessary.
Educate patients and families on care plans and health management.
Document patient interactions and maintain accurate records.
Utilize data to track performance and improve service delivery.
Provide feedback to improve processes and patient care.
Stay updated on industry trends and best practices.
Engage in community outreach to promote health services.
Experience / Qualifications
Valid RN license in Montana with case management certification preferred.
Commitment to ongoing professional development and training.
Proven experience in case management or patient care coordination.
Demonstrated ability to meet performance targets and goals.
Strong communication skills for patient and team interaction.
Ability to work collaboratively within a healthcare team.
Ability to analyze data for performance tracking and improvement.
Familiarity with electronic health records and documentation standards.
Strong problem-solving skills to adjust care plans as needed.
Experience in community outreach and health promotion activities.
