Revital Rehab Services is seeking a dedicated Part -Time / PRN Physical Therapist (PT) to join our care team at

River Terrace , a skilled nursing and rehabilitation facility in Bluffton, IN. This is your opportunity to deliver life-changing rehab care, restore mobility, and improve quality of life—while working in a collaborative, supportive environment that puts clinicians first.

What You’ll Do :

Conduct assessments and develop personalized treatment plans with short- and long-term goals

Deliver evidence-based physical therapy services in accordance with best practices and facility guidelines

Collaborate with an interdisciplinary team to coordinate care and optimize outcomes

Maintain accurate, timely documentation of patient progress and adhere to productivity standards

What You’ll Bring :

Active Physical Therapist license in the state of Indiana

Current BLS certification

Passion for patient-centered, high-quality rehab care
