Job Title : RN Admission Specialist Home Health

Company : Home Health Care, Inc.

Compensation : $150 - $200 per visit + mileage reimbursement!

Take charge of the start of care journey. As an RN Admission Specialist with Home Health Care, Inc. , youll deliver expert OASIS documentation and launch personalized care plans for clients transitioning into home-based care.

Responsibilities :

Perform comprehensive patient assessments and OASIS documentation

Recommend and coordinate additional services

Communicate with providers and caregivers to ensure safe transitions

Qualifications :

Minnesota RN license in good standing

Admission and OASIS experience

Strong communication and time management

Reliable transportation

Relevant MOC Codes :

Army : 66H, 66N

Navy : 1960

Air Force : 46FX

Marines : MED

Coast Guard : HS
