Role Description

This is a full-time on-site role for a Licensed Practical Nurse (LPN) located in Medford, WI. The LPN will be responsible for providing direct patient care, administering medications, recording vital signs, and assisting with patient assessments, skin assessments and wound care. Additional duties include collaborating with healthcare team members to develop care plans, educating patients and their families on health conditions and treatments, and maintaining accurate medical records.

Qualifications

• Proficient in patient care, administering medications, and recording vital signs
• Skilled in assisting with patient assessments and developing care plans
• Excellent communication skills for patient and family education
• Ability to collaborate effectively with healthcare team members
• Strong organizational skills and attention to detail for maintaining medical records
• Current LPN license in the state of Wisconsin
• Basic Life Support (BLS) certification required
• Experience in long-term care or similar settings preferred
