Case Manager - Housing

The Position :

St. Patrick Center, a ministry of Catholic Charities, is hiring a Case Manager to join our Housing program. In this vital role, you'll assist families with immediate housing needs, connect them with community resources, and empower them with life skills for greater stability. Your expertise in crisis intervention and financial guidance will be essential in fostering overall. The case manager will assist with housing placements, conduct monthly in-home visits, and develop individualized treatment plans to empower clients toward self-sufficiency. You'll link clients with essential community services, help them navigate daily living skills, and provide transportation to appointments. A collaborative spirit is key as you work with community partners, attend meetings, and tackle departmental goals. The core working hours are Monday through Friday, 8 : 00am to 4 : 30pm.
