Pennsylvania Licensed Health Care Manager

A company is looking for a Health Care Manager (Remote).

Key Responsibilities

Ensure continuity and coordination of care for members with chronic conditions and complex health needs

Develop and coordinate individualized treatment plans, monitor effectiveness, and make necessary adjustments

Collaborate with various departments to improve services and provide accurate information to members and providers

Required Qualifications

Master's degree in a human service field or a Licensed Pennsylvania RN with 6-8 years of clinical experience

Five years of experience in clinical, care coordination, and / or case management

Previous experience in inpatient, hospice, or home health settings is a bonus

General knowledge of best practices in health care, especially with special needs populations

Registered Nurse (RN) or Social Services Licensure (LSW, LCSW, LPC) required
