- Conducting in-person patient interviews and thorough physical assessments.\n
- Overseeing the implementation and evaluation of the patient's care plan, coordinating with home health aides, LPNs, RNs, and other caregivers.\n
- Communicating patient conditions and collaborating with relevant providers to adapt the care delivered as patient needs change.\n
- Educating patients and families on home health care procedures, such as wound care, IV administration, and medication management.\n
- Working to reduce readmissions by promoting preventive care and ensuring continuity of care.\n\nTo qualify as a Registered Nurse Case Manager (RNCM) with us, you will need : \n
- A current, unrestricted license to practice as a Registered Nurse (RN) in the applicable state.\n
- Current CPR / AED / BLS / First Aid certification.\n
- Reliable transportation for travel to care sites and / or work locations.\n
- At least one (1) year of professional nursing experience in home health or a similar setting; prior case management / utilization review experience is preferred.\n
- OASIS experience is preferred.\n
