As an Admission RN, you will play a crucial role in our patients' journeys. Your responsibilities will include :
Conducting initial assessments of patients’ physical and psycho-social status.Collaborating with the Interdisciplinary Group (IDG), patients, families, and attending physicians to develop personalized care plans.Providing education and interventions to effectively manage pain and other physical symptoms.Ensuring the availability of necessary equipment, supplies, and medications.Adapting care plans as patients’ and families’ needs evolve.Documenting patient and family services accurately and promptly.Participating in regular IDG meetings to enhance patient care.What We’re Looking For :
Registered Nurse : Must hold an active, unrestricted license in the State of Michigan.Experience : Community health and a broad range of Medical-Surgical experience is preferred.Transportation : Reliable transportation with a valid state driver's license and current car insurance.Join us in making a difference in the lives of those we serve.
