This role is a unique opportunity to bring personalized, preventative, and ongoing primary care to patients in the comfort of their homes.

Key Responsibilities

• Provide comprehensive  primary care services  in patients' homes, including routine health maintenance, chronic disease management, preventative care, and acute issue management.
• Conduct physical examinations, order and interpret diagnostic tests, and prescribe medications as appropriate.
• Develop and manage individualized care plans in collaboration with patients, families, and interdisciplinary care teams.
• Coordinate care and referrals with specialists, hospitals, and community resources.
• Educate patients and families on health management, preventive care, and treatment options.
• Document all patient encounters accurately and timely in the electronic health record.
• Travel within the  Chicago area  to patient homes, with scheduling supported by our operations team.
