Summary :

The Lead Case Manager - Medical Outreach role is responsible for acting as the senior Case Manager on the team to be used as a resource and guide to all other team members. In addition to the functions of a Medical Outreach Case Manager carrying a caseload, the Lead is the main point of contact for management when dealing with matters of budgeting, staffing, strategizing, and problem solving.

Required Minimum Knowledge, Skills, and Abilities (KSAs)

• Education : Bachelor's Degree preferred, with a specialization in Psychology, Behavioral Health, Social Work, Special Education, Counseling, Rehabilitation, Nursing, or closely related field preferred.
• License / Certifications : Current applicable state(s) license as a Licensed Social Worker preferred. Valid driver's license and reliable vehicle required.
• Experience : With a Bachelor's Degree : two years of experience in a community health setting, including one year of case management experience required. In lieu of Bachelor's Degree : Three years of directly relevant experience in a healthcare setting, including 6 months of case management experience. Experience collaborating with state and federal agencies, community agencies, municipal, and public programs preferred.
