Position Overview

We are seeking a compassionate and reliable  Licensed Practical Nurse (LPN)  to join our dedicated care team on the  2nd or 3rd shift . The ideal candidate will deliver quality nursing care under the supervision of a registered nurse or physician, ensuring exceptional outcomes and comfort for our residents.

Key Responsibilities

  • Administer prescribed medications safely and accurately
  • Record and monitor resident vital signs and health status
  • Support resident assessments, documentation, and care planning
  • Collaborate closely with physicians, RNs, and CNAs to meet resident needs
  • Provide both physical and emotional support to residents and their families
  • Qualifications

  • Active  LPN license  in the State of Wisconsin
  • Strong communication, organization, and teamwork skills
  • Comfortable working in a fast-paced, collaborative environment
  • Basic computer proficiency for documentation and charting
  • Knowledge of medical terminology and nursing procedures
