You'll have the freedom to shape your schedule, deliver individualized care, and help patients regain independence in the place they feel safest - their home.

🏡 What You'll Do

As a Home Health OT, you'll :

Provide personalized, in-home therapy services for a wide range of patients

Assess functional limitations and create individualized care plans

Conduct home safety evaluations and recommend adaptive equipment

Collaborate with nurses, PTs, caregivers, and family members for holistic care

Track patient progress with clear, timely documentation (EMR training provided)

Travel locally throughout Mesa County ( mileage and time reimbursed )

🔍 Who We're Looking For

Licensed Occupational Therapist (OTR) in Colorado (required)

Home health experience preferred, but new grads welcome - we train and support!

Strong communication, organization, and time-management skills

A passion for helping others live independently and with dignity

Valid driver's license, reliable vehicle, and proof of insurance
