BJC Home Care offers patients and their families a complete range of home care services, including skilled nursing services, adult and pediatric hospice and supportive care, rehabilitation therapy, home infusion therapy, infusion treatment rooms, home medical equipment and high-tech respiratory care. Specialty home care programs also are available, including adult and pediatric asthma, cardiac, diabetes, orthopedic and wound care programs.

The Metro St. Louis Intermittent Home Care Department of BJC Home Care Services provides home visits to patients in the metropolitan St. Louis area and several nearby counties, with 24 hour on-call home care nursing supervision. Role Purpose
The Home Health Manager is responsible for managing the activities of the home health agency. This position will coordinate and complete assigned projects to effectively support the immediate and long range objectives of the organization; oversight of the eligibility of patients referred to home health services, planning for the services to be provided to patients and supervising their total care; implementing and maintaining administrative practices; enhancing the profitability of the agency; and providing motivation and retention of a qualified staff and assure the quality of services delivered. This position also acts as the liaison with leadership and other departments throughout the organization.
Responsibilities
Ensures the highest quality of care is provided and is compliant with federal, state and local regulatory requirements and established departmental policies and procedures. Ensures patients receive quality care by reviewing documentation, making visits with staff, reviewing care plans, conducting / participating in multidisciplinary team conferences, and reviewing patient satisfaction reports. Investigates and resolves patient / family member concerns regarding patient care.
Monitors the quality of service and utilization standards and assumes specific responsibility for patient care at the agency level. Ensures coordinated plans of treatment, customer focused care, and cost-effective utilization of services.
Responsibility for overall supervision of home care multidisciplinary team, includes interviewing, hiring, monitoring assignments, and evaluating personnel. Develops and implements action plans to improve staff development.
Participates in program management, including licensing, budgeting, utilization and quality assessment / improvement / activities
Recommends, develops and updates strategic business plans to support organizational practices and goals. Works with health care providers outside of the agency to achieve optimal patient care across the continuum.
