Position Summary : As part of the Critical Time Transition Program (CTTP), the Case Manager provides intensive Critical Time Intervention (CTI) services for youth ages 11–17 with complex mental health needs and their families / caregivers, supporting transitions from higher levels of care to community settings. CTI is a phased, time-limited care management model emphasizing early and assertive engagement, trauma-informed practice, strengths-based approaches, resilience, psychiatric rehabilitation, positive behavioral strategies, and family / caregiver skill-building. Case Managers work across the community and a Transitional Residential Setting (homes, schools, hospitals, outpatient clinics), maintaining frequent contact with youth, families / caregivers, and involved agencies. Services prioritize in-person engagement. Case Managers actively coordinate care in the community, support skill-building, and strengthen family and youth self-advocacy while facilitating safe and effective transitions to the lowest appropriate level of care.

Responsibilities :

  • Carry a caseload of approximately 8 youth and families
  • Provide weekly, phased contacts with youth and families / caregivers, including assertive engagement, linkage to services, monitoring, and skill-building
  • Develop individualized, person-centered care plans in collaboration with the Clinician.
  • Facilitate coordination with hospitals, community providers, state agencies, and informal support.
  • Support youth and families in scheduling and attending appointments; participate in warm hand-off meetings.
  • Assist youth and families with applications for services
  • Provide crisis management, positive behavioral support, and advocacy to youth and families.
  • Collaborate with the Program Director and team members during weekly meetings, agency case conferences, and case reviews to ensure fidelity and continuity of care.

Requirements

Education :

  • Bachelor's degree in human services or related field  required
  • Master's in Social Work preferred
  • Experience :

  • At least 3-5 years working with youth who have experienced mental health challenges, co-occurring disorders, and or intellectual disabilities, community-based interventions required
  • Driver's License and ability to maintain insurability throughout employment required
