About The Position

Job Title : Certified Occupational Therapy Assistant

Profession : Rehabilitation Therapy

Specialty : COTA (Certified Occupational Therapy Assistant)

Duration : 13 weeks

Shift : Monday to Friday, Days

Hours per Shift : 32 hours guaranteed

Experience : Minimum years of experience preferred; new graduates are welcome.

License : Valid state license required.

Certifications : Must provide license verification.

Must-Have : SNF experience preferred.

Description :

Either a Certified Occupational Therapist or Certified Occupational Therapy Assistant may take on a Therapy Coordinator role.

It is essential that the prescreen form is completed and attached upon submitting a candidate.

A resume and license verification also must be attached.

Failure to complete and attach these items will result in candidate decline.

A Covid test is required 72 hours prior to the start date, with results issued 48 hours before the start date.

Please review the credentialing checklist prior to submission.

State license verification is necessary, along with a resume and three references.

A county criminal background check for seven years for all counties lived and worked is required.

A federal criminal background check for seven years or more is also necessary.

A multi-state criminal search for seven years or more is required.

A nationwide sex offender search for seven years or more must be conducted.

HHS / OIG and GSA searches are essential.

A social security number trace is needed, alongside either a step PPD or Quantiferon, or proof of a positive PPD and chest X-ray.

A SAM search is required.

Oklahoma state background checks with fingerprints are also necessary.

Position urgency is normal.

Holiday coverage is not required.
