Hospice RN Case Manager (RNCM)

Location : Palos Heights, IL

Employment Type : Full-Time (40 hrs / week)

Compensation & Benefits

Base Salary : $80,000–$85,000 annually (based on experience)

Starting Bonus : $10,000 First Year Success Bonus

Relocation Assistance : Not offered

Shift : Monday–Friday, 8 a.m.–5 p.m. (with variable start / end times)

On-Call Rotation :

(1) 24-hour weekend day per month (Saturday or Sunday)

• (2) weekday evening on-calls per month (5 p.m.–8 a.m.)
• (2) holidays per year (1 summer, 1 winter)
• Dedicated After-Hours RNs provide additional support nightly
• Additional compensation for on-call duties

We’re looking for a Registered Nurse Case Manager (RNCM) with a heart for hospice and a head for organization. In this role, you’ll manage patient care, coordinate with interdisciplinary teams, and support families through the hospice journey — ensuring every patient receives the dignity and compassion they deserve.You’ll be out in the community, covering a 25–30 mile radius around Palos Heights, IL.
