Description :

The Hospice RN provides skilled nursing care to patients in a home health or hospice setting. The nurse collaborates with the interdisciplinary team to ensure high-quality patient care, symptom management, and family support. Experience with HCHB documentation is highly valuable.

Requirements :

Active CT RN license in good standing

Home Health experience required

Hospice experience preferred
