Job Summary
We are seeking a compassionate and motivated Licensed Practical Nurse II (LPN II) to join our healthcare team. The LPN II provides direct and indirect nursing care to an assigned group of patients based on physician orders and established Nursing Care Plans. This role works closely with Registered Nurses to coordinate patient care, perform nursing procedures, collect and document patient data, and ensure quality standards are met.

Key Responsibilities
Perform nursing skills appropriate for the patient population and specialty area.Gather patient data, implement care, and evaluate outcomes based on physician orders and nursing care plans.Accurately take, note, and follow physician's orders.Collaborate with Registered Nurses to plan patient care.Document patient data, nursing process, progress, and outcomes clearly and accurately.Provide patient and family education, including discharge planning.Make appropriate referrals and coordinate follow-up care as needed.Ensure compliance with unit and hospital standards of care, including cultural, psychosocial, and age-specific considerations.Attend mandatory in-service training and meet unit education requirements.Complete variance / incident reports accurately when required.Contribute to quality improvement (PI) initiatives and educational activities.Demonstrate teamwork and effective communication with patients, families, internal staff, and external providers.Decision-Making & Work Environment
Carry out non-routine procedures under changing conditions, following supervisor guidance.Lead peers by example in delivering patient care (not a supervisory role).Work directly with patients, families, internal staff, and external healthcare professionals in both face-to-face and telephone interactions.Required Skills & Experience
0–6 months of related nursing experience (entry-level friendly).Working knowledge of general disease management processes, bio-psychosocial models, and clinical pathways.Strong interpersonal, oral, and written communication skills.Ability to organize, prioritize, and perform duties efficiently.Attention to detail, especially when handling clinical and numerical data.Education & Licensure
Required Education : Completed LPN program / degree.Required Licensure : Valid multi-state or Oklahoma Licensed Practical Nurse (LPN) License.Required Certification : Current BLS (Basic Life Support).Preferred Qualifications
