Shift Structure

  • Typical rounding hours : 7 : 00 AM – 1 : 00 PM local time, addressing patient and facility needs.
  • Flexible start times around 7 : 00 AM, depending on workflow.
  • Monday–Friday schedule — no weekends or holidays required.
  • Charting may be completed remotely after on-site rounds.
  • In-person position based within skilled nursing facilities.
  • Essential Responsibilities

  • Must be Medicaid-enrolled in the state of employment.
  • Provide and oversee the daily clinical care of residents within assigned skilled nursing facilities.
  • Conduct comprehensive histories and physicals, diagnose, and develop treatment plans — including diagnostics, prescriptions, and monitoring.
  • Offer patient counseling and education on health maintenance and disease prevention.
  • Deliver high-quality, patient-centered primary care within the SNF environment.
  • Complete accurate, timely documentation of all encounters.
  • Be available for phone or secure messaging consultations with nursing staff as needed.
  • Use facility EHR systems such as PointClickCare, MatrixCare, and American HealthTech (training provided).
  • Maintain reliable transportation and punctual attendance for all scheduled shifts.
  • Collaborate with supervising physicians when applicable, following established clinical protocols.
  • Participate in chart reviews and quality audits as part of ongoing clinical improvement.
