Position Overview :
We are seeking a detail-oriented and organized Chronic Care Coordinator (Medical Assistant) to join our care coordination team, focusing primarily on documentation upload and chart review, with occasional support for patient phone calls as needed. This non-clinical, remote role is ideal for medical assistants who are experienced with chronic care documentation and comfortable using electronic health record (EHR) systems.
Key Responsibilities :
Accurately upload and manage patient documentation within EHR systems
Conduct comprehensive chart reviews to ensure accuracy, completeness, and compliance
Identify and report missing or outdated documentation to the appropriate care team members
Provide occasional coverage for patient phone calls, such as follow-ups or care coordination support
Communicate professionally with patients when needed to support continuity of care
Collaborate with clinical staff to maintain up-to-date patient records
Ensure full compliance with HIPAA and internal confidentiality standards
Qualifications :
2+ years of experience as a medical assistant
Previous experience with chart review, documentation, or care coordination.
Proficiency with EHR / EMR systems
Strong attention to detail, organization, and time management
Excellent written and verbal communication skills
Ability to work independently and manage remote workflows
Comfortable handling occasional patient phone interactions professionally
Knowledge of CMS chronic care management billing codes
HIPPA compliance and patient confidentiality
