Monday – Friday, 8am-5pm Rotating and flexible on-call schedule.We use eClinical Works (eCW), along with partnering programs to ensure ease of use for our patients and practices.We focus on continuity of care, health maintenance, and disease prevention, working alongside patients and their families to prevent, diagnose, treat and manage a population with multiple comorbidities.Integrated team-based care :
Working as a multidisciplinary team to improve patient outcomes by meeting their needs and preferences.Prioritizing preventative care and proactive management of chronic conditions to improve patient outcomes, decrease patient’s costs, and increase revenue for the practice.Providers must be comfortable managing a primarily adult-aged patient population.Applicant Requirements :
Certification through the American Academy of Nurse Practitioners (AANP) OR American Nurses Credentialing Center (ANCC) Certification through the American Academy of Nurse Practitioners (AANP) OR American Nurses Credentialing Center (ANCC) program, focusing on adult medicine, Certification through an accredited PA certification program focusing on adult medicine.Currently licensed as an Acute, Adult / Gerontological or Family Nurse Practitioner in the state of practice or the ability to obtain a license prior to employment Holds or is eligible for DEA licensure and CDS licensure (if required in the state of practice) Up-to-date CPR Certification and Basic Cardiac Life Support certification.Strong organizational and prioritization skills.Ability to use Electronic Medical Record (EMR) system Knowledge of Medical Advantage, HEIDS, MRA and ICD-10 Coding Comfortable managing a primarily adult-aged patient population Demonstrated ability to problem-solve complex and multifaceted situations. #IND Powered by JazzHR
