The Home Care Supervisor is under the direct supervision and direction of the Regional Director of Nursing, as well as the Home Care Director if applicable for branch / region. The Home Care Supervisor oversees assigned home care patients, Home Care Nurses, Home Care Aides, and Personal Care Aides. The Home Care Supervisor works collaboratively with all departments to provide high quality care in a safe environment. The Home Care Supervisor adheres to all Federal, State, Local and Company policies and regulations. Essential Job Responsibilities :
- Makes initial contact with the patients and explains services offered
- Completes start of care assessments and documentation to onboard new patients
- Designs an individualized plan of care for each patient
- Instructs assigned care team members on the needs and care plan details
- Reports progress of care to parties involved
- Follows up and maintains relationships with clients, always advocating on their behalf
- Establish relationships with community partners and referral sources
- Keeps Clinical and Operational abreast of patient condition and / or medical changes
- Reviews of caregiver documentation
- Take part in interdisciplinary team meetings to ensure needs of all patients are met
- Supports organizational growth during mergers and acquisitions as needed
Follow all duties as described under the state regulations for the company's Home Care Licensure, as well as maintain standards appropriate to state / regional licensure, contracts, and OSHA. The duties of the Supervisor shall include :
Knowledge of OSHA, DMAS, CCC Plus Program, and State Licensure Regulations governing home care and nursing practiceConduct orientation and training or supervising the orientation and training of agency employees assigned to the nursing service as directedAssist with the assigning of RNs, LPNs, Certified Nursing Assistants / Personal Care Aides and other nursing support staff to individual patientsAssist in Safety Training and general training needs of employees as assignedPerform initial in-home evaluation as assigned of patients requiring services of a registered nurse, licensed practical nurse, home care aide, personal care aide, or other nursing support staffInitiate the patient's plan of care in accordance with nursing needs specified in the approved plan of treatmentDeliver or supervising the delivery of skilled nursing servicesRevise patient's plan of treatment if skilled services are involvedInitiate preventative and rehabilitation nursing services through work with community partnersPrepare and coordinate clinical and progress notes on nursing services deliveredIn-home supervision of nursing services delivered by licensed practical nurses, home care aides, personal care aides and other nursing support staffIf the agency contracts with individuals to deliver registered nurses, licensed practical nurses, home care aides, personal care aides, or other nursing support staff, the agency nurse supervisor shall be responsible for conducting in-home supervisory visitsIn-home supervisory visits shall be made when the registered nurse, licensed practical nurse, home care aide, personal care aide, or other nursing support employees being supervised are present and delivering services to the patientReporting changes in the patient's medical or mental condition to the attending physician, care team as necessary, and individuals involved in the patient's careTeach or supervise the teaching of household members who are responsible for assisting the patient with his / her nursing or personal care needsParticipate in quality improvement reviews, as requestedOther duties as assignedRequirements :
Must possess an active state Registered Nurse license for the state they will be working in (compact / multi state license is acceptable)Minimum of 1 year in a Nursing role- LPN to RN acceptableMinimum of Associates Degree in NursingComfortable working collaboratively in cross-functional teamsAbility to communicate effectively, both verbally and in writing, with all levels of the organizationExcellent presentation skillsAdequate transportation for home visits with mileage reimbursementComputer / iPad Technology skills for documentation purposesAble to travel on a regular basisBenefits :
