Position : Field Nurse Practitioner (Boise, ID)\n\nJob Description : \n\nThe Care Connections Nurse Practitioner focuses on screening and preventive primary care services delivered in the home, community, and nursing facility settings. They provide medical care in environments where patients feel most comfortable and are most receptive, including home, nursing facilities, and pop-up clinics.\nJob Duties\n
- Provide general medical care and care coordination to various and / or specific patient levels including adults, women's health, pediatric, and geriatric.\n
- Perform comprehensive evaluations including history and physical exams for gaps in care and preventive assessments.\n
- Address both chronic and acute primary care complaints and determine medical urgency.\n
- Establish and document reasonable medical diagnoses.\n
- Seek specialty consultation as appropriate.\n
- Order / perform pertinent diagnostic laboratory and radiology testing; use tests judiciously in a limited-resource environment.\n
- Know when patient needs are beyond scope and when physician oversight is required.\n
- Create and implement a medical plan of care.\n
- Schedule patient appointments as appropriate.\n
- Provide post-discharge coordination to reduce hospital readmission rates and emergency-room utilization.\n
- Perform face-to-face visits in a variety of settings, including home, skilled nursing facilities, and public locations.\n
- May perform face-to-face visits via alternate modalities based on business need, leadership direction, and state regulations.\n
- Order bulk laboratory orders targeted to specific member populations.\n
- Perform on-call coverage to triage urgent lab results and pharmacy inquiries and develop an appropriate plan of care.\n
- Participate in community-based pop-up clinics to build relationships and address gaps in health care.\n
- Drive up to 120 miles a day on a regular basis to a variety of locations; occasional drives beyond this may be required for special projects.\n
- Obtain and maintain cross-state licenses as needed for business requirements.\n
- Collaborate with fellow nurse practitioners to develop best practices for efficient and effective work.\n
- Actively participate in regional meetings.\n
- Prescribe medications and perform procedures as appropriate.\n
- Perform timely documentation in an electronic medical record system.\n
- Occasionally walk flights of stairs while carrying up to 50 lbs. of equipment.\nJob Qualifications\n
- Required\n\nEducation : \n\nMaster's degree in family health from an accredited nursing program.\n
- Required Licenses &\n\nCertifications : \n\nAn active and unrestricted national certification from the American Academy of Nurse Practitioners, American Nurses Credentialing Center, or similar organization. Current state-issued license to practice as a Family Nurse Practitioner. Current Basic Life Support for Healthcare Professional certification. Current unrestricted driver's license.\n
- Required Skills : Advanced computer skills; proficient with Word, Excel, and an electronic medical record system.\n
- Preferred Experience : 3-5 years as a Registered Nurse and / or Nurse Practitioner, ideally in home health, community health, or public health. Experience managing chronic conditions in home health. Experience with underserved populations facing socioeconomic barriers to health care. Fluency in a language other than English is a plus. Immunization and point-of-care testing skills.\n\nCompensation Range : $88,453 - $206,981 / ANNUAL.
