Respite Qualified Disability Professional (QDP)
Schedule : Sunday - Thursday, Varies
Location : Bowling Green, OH
Hourly Rate : $22.69
Type : Full-Time
Job Summary :
QDP applies Division policies and exercises independent judgment in supervising staff and managing
training programs. The role requires minimal supervision except for unusual situations, with work reviewed periodically
for compliance and effectiveness. QDP regularly coordinates with Easterseals staff, external agencies, officials, clients,
and families to ensure training operations meet client needs.
Key Responsibilities :
Trains and assists direct support professionals in preparing for local, state and federal reviews
Ensures that services adhere to regulations.
Maintains detailed client case notes, compiles and submits reports.
Is accountable for client finances. Accurately tracks, monitors and documents all approved billing
Ensures accurate and proper documentation of services; ensures that services adhere to regulations
Schedules and facilitate team meetings to assist in the development of the Individual Support Plan (ISP) and person
centered plan.
Monitors for progress and ensures follow through with objectives listed in the individual's ISP and BSP
Applies for and refers to community resources to meet the emotional, financial, and vocational and education, legal and
social needs of the individual
Provides crisis intervention / supportive counseling, assists with supervision of clients as needed, participates in on-call
Completes weekly site visits to ensure that all the individuals' plans are implemented as written, documentation in the
home is correct and to ensure that the individual's needs are being met
Supervises assigned personnel, including interviewing, hiring, orientation, training, disciplining and terminating,
reviewing and approving work schedules.
Adheres to Easterseals Human Resources and EEO / AA policies and perform other duties as assigned
Provides staff training on each participant's current ISP, BSP and high risk plans. Ensures that all staff are properly
trained prior to working with participants and that the training is documented according to department policy.
Coordinates medical services for individuals including hospice, hospitalizations, rehabilitation and routine medical
services, providing transportation as needed
Arranges for special medical services, including hearing aids, therapies and orthopedic shoes, adaptive equipment etc.
Ensure ordering medication refills as needed
Monitors MAR's, ensure MAR documentation is correct and complete
Uphold agency values and follow agency policies and procedures
Maintain confidentiality of records / information according to HIPAA, State, Federal Laws and guidelines
Maximize and responsibly utilize agency and community resources
Serves as liaison to participants, families and community resources
Serves as liaison to State and local agencies
Qualifications & Experience :
Must be 18 years of age
High School diploma or GED
Experience working with individuals with autism and other developmental disabilities; prefer 2 years
1 year of supervisory experience preferred
Ability to pass criminal registry, background checks and other checks deemed necessary
Valid driver's license and insured vehicle
Basic language, written and computer skills necessary for communication and documentation
Ability and willingness to support individuals with challenging behavior and or medical concerns
Ability to pass and maintain all required training deemed by DODD
Work Environment / Conditions
This position requires regular in-county travel, public speaking, and occasional evening or weekend work. Work is primarily inoffice or in the field within Wood County, with some virtual meetings. Independent work is expected, with flexibility to adapt to
community needs and organizational priorities. Occasional travel to corporate office is expected. Occasional lifting may be
required.
