Work Schedule : Full-time or Part-time - Variable Shift based on 90% productivity
Pay Range : $40 - $50 / Hour depending on experience
JOB SUMMARY : The Speech Language Pathologist reports to the manager / director and performs speech language pathology services (both evaluation and treatment) as assigned by that manager / director.
JOB DUTIES :
Perform patient evaluations and develop treatment plan, including short-term and long-term goals, consistent with physician orders and appropriate for the patient’s overall medical condition.
Carry out patient care plans or portions thereof, determining appropriate progression of patients within the realm of the established care plan.
Perform tests and complex treatments individually or with assistance as needed.
Collaborate with occupational therapists, physical therapists, physicians and other members of the healthcare team relative to individual patient treatment programs and progress.
Document patient evaluations, treatment progress notes, and discharge summaries for the patient’s medical records in a timely manner as defined by the department policy manual.
Providing a full range of speech therapy modalities consistent with previous education, experience and clinical needs.
REQUIREMENTS - EDUCATION / LICENSURE / EXPERIENCE :
Master's degree in Speech-Language Pathology from an accredited program
Current state licensure or in the process of licensure in this state
Current basic Life Support certification in accordance state regulations
Must be clinically driven, dependable, and self-motivated
Can adjust with changes in medical field with according to CDC guidelines.
Should have as a minimum, 1 year experience in a skilled nursing facility, preferred
Must possess the ability to make independent decisions, follow instructions and accept constructive criticism
Must possess leadership ability and the willingness to work harmoniously with other personnel.
Must possess good judgement and can maintain highly confidential information.
Must be knowledgeable of HIPAA compliance and requirements.
Licensure : Colorado SLP licensure and Certificate of Clinical Competence from the American Speech-Language Hearing Association.
Open to new graduates as well as students needing fellowship experience.
