Medical Assistant, Facility Liaison – Make a Real Impact!
LifeCircles PACE is seeking a caring and proactive Medical Assistant to join our team as a Facility Liaison . In this unique and rewarding position, you’ll be the bridge between our interdisciplinary care team and our contracted residential facility partners, ensuring participants receive seamless, high-quality care that supports their medical, physical, and emotional well-being. This role combines both clinical and administrative tasks performed at the LifeCircles Day Center, contracted facilities, and in participants' homes.
Primary Responsibilities :
Serve as a primary point of contact to facilitate communication between LifeCircles and residential facility partners.Perform clinical duties including taking medical histories, obtaining and documenting vital signs, collecting and preparing laboratory specimens, administering medications or immunizations as directed by a medical provider.Collaborate with internal clinical teams and external providers to coordinate care.Track appointments, labs, and follow-ups, ensuring timely and accurate information exchange.Ensure orders, recommendations, and care updates from LifeCircles are received and understood by facility partners.Regular collaboration with CNAs, nurses, medical providers, and other members of the interdisciplinary team to ensure participant’s clinical needs are met.Monitor adherence to care plans and identify deviations or risks.Perform administrative duties including coordinating transportation, performing data entry, answering phone calls, and coordinating with outside providers / consultants.✅ What You’ll Need :
Certified or registered Medical Assistant in the state of Michigan from an accredited program, required.Must possess a current, valid driver’s license, reliable transportation, and maintain proper auto insurance.Experience as a caregiver, preferred.One year of experience working with the frail elderly, long term care population, or end-of-life care, preferred.Prior experience with phlebotomy, preferred.CPR certification or willingness to obtain upon hire.High school diploma or GED required.The above is a summary of the position, it in no way states or implies that these are the only duties this position will be required to perform.
