You'll report directly to the Executive Director or Administrator and be assigned to a location to conduct Face-to-Face (F2F) visits that support the clinical eligibility of patients receiving hospice care.

Your focus will be performing physical assessments and clinical documentation that supports the hospice physician's certification or recertification of a patient's terminal diagnosis, in accordance with CMS requirements.

As a Hospice Nurse Practitioner, You Will :

Perform and document comprehensive physical assessments during required Face-to-Face encounters

Communicate clinical findings to the hospice physician responsible for certifying the patient's terminal prognosis

Review and interpret diagnostic test results, medication profiles, and treatment plans to support hospice eligibility

Adhere to the CMS-mandated timeframe for F2F assessments

Ensure all documentation meets hospice regulatory standards

About You

Requirements :

Master's or higher degree in Nursing

Graduate-level Nurse Practitioner program completed through an accredited institution

Current RN license in the state of requested employment

Current state license or certificate as an Advanced Practice Registered Nurse (APRN)

National certification as a Nurse Practitioner through ANCC or AANP

Must have an active collaborative agreement in place, as required by state

Preferred Experience :

Previous experience in hospice, palliative care, or geriatrics

Strong understanding of Medicare hospice regulations and eligibility criteria

Experience providing care in home-based or facility settings

Prior experience completing F2F or recertification visits in hospice or home health

Confident, independent practitioner with excellent communication and documentation skills
