Developmental Disabilities Waiver Case Manager

Role Description

This is a full-time, on-site role for a Waiver Case Manager located in the Greater Minneapolis-St. Paul Area. The Waiver Case Manager will be responsible for coordinating services and resources for individuals with developmental disabilities, conducting assessments, developing support plans, and ensuring individuals receive appropriate services. Daily tasks include managing case files, providing advocacy, monitoring individuals goals and progress, and collaborating with other service providers. The role also involves communicating with the individual's families and maintaining accurate documentation.

Qualifications

• Experience in case management, social work, or a related field
• Excellent organizational and time management skills
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Cultural competence and sensitivity to diverse populations
• Bachelor’s degree in Social Work, Human Services, or a related field
• Familiarity with local, state, and federal social services programs
• Proficiency in computer applications and case management software
• Authorized to work in the USA
• Valid Driver's License and record
• Background Check Clearance and Employment Verification
