Work schedule / hours : Monday - Friday, 8 a.m. - 5 p.m.

Job Summary :

Provides oversight and coordination of care for members across the continuum of care while adhering to professional standards of case management practice and meeting regulatory standards.

Minimum Qualifications :

Bachelor's degree (BA / BS) from four-year college or university; or one to two years' related experience and / or training; or equivalent combination of education and experience.

Licensed Medical Professional required. C ertified Case Manager credential is required within 12 months of hire or appointment.

Must be able to travel between locations and perform occasional after hours, weekend and holiday work.
