Hospice PRN F2F Nurse Practitioner

Qualifications : Current, unencumbered Nurse Practitioner (NP) license in the state of Illinois. Minimum of 2 years of clinical experience; hospice or palliative care experience a plus. Strong assessment, communication, and documentation skills. Ability to work independently, manage time effectively, and adapt to a PRN schedule. Valid driver’s license, auto insurance, and reliable transportation (travel required to patient homes and facilities). HOURS : PRN Monday-Friday 2-3 Days / Week - Shift : 8 : 30am-5pm - NO WEEKENDS NO HOLIDAYS TERRITORY : Deerfield, IL. Perform patient assessments focused on clinical status, disease progression, and functional decline. Document findings in accordance with CMS regulations and agency policies. Collaborate with interdisciplinary team members, including physicians, nurses, social workers, and chaplains, to support care planning. Communicate assessment outcomes clearly and effectively with both patients / families and care teams. Provide clinical insight and education regarding disease trajectory and hospice appropriateness.
