Role Description

This is a full-time, on-site role for a Registered Nurse located in Wytheville, VA. The Registered Nurse will be responsible for providing comprehensive in-home health care services to patients. Daily tasks include assessing patient health, developing and implementing nursing care plans, administering medications, and coordinating with other healthcare professionals to ensure consistent patient care. The Registered Nurse will also educate patients and their families on how to manage their health conditions.

Qualifications

• Strong clinical skills and knowledge of nursing care practices
• Experience in home health care, hospice, or community-based services
• Excellent communication and patient education skills
• Ability to coordinate care with a multidisciplinary team
• Active RN license in Virginia
• CPR certification is required
• Valid driver's license and reliable transportation
• Experience in value-based healthcare is a plus
• Bachelor's degree in Nursing is preferred
