Community Health Worker Job Description
Our organization seeks a skilled Community Health Worker to bridge the gap between patients, community resources, and healthcare services. This role is critical in removing barriers and increasing wellness for our patients.

This is a full-time, hourly position that requires spending a balanced time in the field and in our clinics. As a key member of our High Risk Care team, you will work with patients to identify personal health goals, support care team decision making, and coordinate clinical and complementary services needed to provide high-quality healthcare.
Key Responsibilities :
- Establish and maintain strong interpersonal relationships with patients, community organizations, team members, and partners to support and coordinate services for patients.
- Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services.
- Manage resource-focused care plans as a component of a patient's continuum of care.
- Form relationships with and build an inventory of local community organizations that may benefit our patients.
- Connect patients to state and local community resources related to housing, transportation, food, and activities of daily living among other social and physical barriers to health.
- Assist patients with completion of applications for accessing eligible benefits and resources.
- Promote goal setting and achievement to improve patients' quality of life and self-efficacy with patients.
Required Skills and Qualifications
To be successful in this role, you must possess :
At least 2 years of patient-facing work experience in social services, community-based case management, or healthcare outreach programs.Strong verbal and written communication skills; Spanish speaking preferred.Proficient computer skills (Google Workspace - Docs, Sheets, Slides) as well as experience utilizing electronic medical record systems.Must have access to an independent means of safe and reliable transportation as well as the ability to travel to various locations in the community throughout the day; mileage will be reimbursed at the IRS / Federally established rate.Benefits
