Job Summary :

We are seeking a compassionate and skilled Physical Therapist (PT) to join our team. As a Physical Therapist, you will manage the complete therapeutic care of an assigned group of patients, coordinating with physicians and the patient care team to ensure the best outcomes. You will focus on individualized care plans aimed at achieving patient goals, enhancing engagement, and ensuring a positive patient experience. Your role will support patients through the continuum of care, delivering high-quality, patient-centered services.

Key Responsibilities :

• Patient Evaluation and Treatment : Provide evaluations and skilled therapeutic interventions in accordance with the scope of practice and professional ethics.
• Therapeutic Assessments : Assess and evaluate the rehabilitative and functional status of patients, contributing to the development of their individualized care plans.
• Care Implementation : Deliver purposeful therapy interventions to produce positive changes in patients’ conditions and functions, aligned with the physician's plan of care.
• Patient and Family Education : Instruct patients, families, and caregivers on the use of therapeutic exercises, appliances, and / or prosthetic devices for use outside of therapy sessions.
• Caseload Management : Prioritize patient caseloads, determine staffing needs, and coordinate daily assignments and patient handoffs to ensure effective care delivery.
