Patient Enrollment & Education : Introduce patients and caregivers to the Virtual Care program, review treatment plans, and empower families with self-care techniques. Collaboration : Partner with physicians, care coordinators, social workers, and inpatient teams to ensure smooth discharges and continuity of care. Patient Advocacy : Identify barriers to care, address social needs, and work to prevent avoidable readmissions or hospital visits. Innovation & Growth : Actively contribute to performance improvement initiatives, staying ahead of industry best practices. Education : Associate’s Degree or Nursing Diploma required; BSN preferred; MSN strongly preferred. Licensure : Active RN license (Single State or Multi-State Compact). Certifications : BLS required; ACLS preferred within 180 days. You’ll complete in-home assessments, enroll patients into the program, and collaborate with a dynamic care team to ensure patients receive the highest quality support outside the hospital walls.
